Health Equity and Minority Health 

Minority Health and Health Equity Liaison

The Minority Health and Health Equity Liaison for Wakulla County aids the Florida Department of Health Office of Minority Health and Health Equity in advancing health equity and improving the health outcomes in the areas of racial and ethnic minorities and other vulnerable populations, to help improve the social determinants of health in the county. The Minority Health Liaison helps to facilitate health equity discussions, initiatives and collaborations with other community members to improve the county overall.

Health Equity Plan

The health equity plan was created to guide others into improving and/ or creating strategies to address systems and increase opportunities to achieve health within our priority populations. A main goal of this plan is to collaborate with local partners and utilize county-level data to improve the social determinants of health in the county.
